ApplicationMembership registration forms are available at the library and on-line (you may download the form and bring it to the library, along with 2 pieces of current identification with your name and address on it * proof of name and address may be the same document if the identification has a photograph otherwise 2 documents are required * proof of address can be your Valid Ontario Driver's License, Ontario Motor Vehicle Permit Property Tax Bill, Age of Majority Card, Old Age Security Card, etc * proof of name can be your Birth Certificate, Valid Ontario Driver's License, Passport, Social Insurance Number, Student ID Card, etc * children under 14 require a parent's signature and the parent must be present when the child receives the card * always bring your card to borrow material * cards are non-transferable
