| Description | Children's Treatment Network (CTN) works with Network partners to deliver comprehensive care and coordinated services to children and youth with multiple special needs * Network partners from healthcare, education, recreation, social and community service organizations have joined together so they can take a team approach to a child's care - at home, in the classroom and throughout the community * families can contact CTN ACCESS at 1-866-377-0286 to discuss their child's needs and receive information about appropriate services and/or visit www.ctnsy.ca ( resource site) for posting, discussions or sharing information with other families *or join us on Facebook. CTN also provides the intake for the new Applied Behaviour Analysis (ABA) Program for children and youth with Autism; Applied Behaviour Analysis Services (ABA) York/Simcoe - Children must have an official ASD Diagnosis, be 18 years old or under and live in York or Simcoe * parents or professionals may download the ABA intake form on the CTN website, complete and fax with the diagnostic report to 705-792-2775; Service Navigation - Assist families to access the appropriate services for children / youth with special needs * a comprehensive assessment of needs and strengths may be completed in complex situations for linking to appropriate services and supports * referral by parents 1-866-377-0286 and/or professionals can fax "Referral for Local Team Services" form to 705-792-2775; Feeding Supports - Continuum of assessment and treatment interventions for children with significant feeding and swallowing difficulties * no urgent care support around feeding * referral by parents 1-866-377-0286 and/or professionals can fax "Referral for Local Team Services" form to 705-792-2775; Inclusive Recreation - Promote and facilitate the inclusion of children who have multiple special needs through recreational opportunities * parents are encouraged to contact their local municipality for suitable and available recreation programs first * referral by parents 1-866-377-0286 and professionals can fax "Referral for Local Team Services" form to 705-792-2775; Occupational Therapy / Physiotherapy - Children and Youth with multiple special needs who have significant mobility issues (use Aid, wheelchair) and or children and youth with a degenerative condition that will affect mobility * fax "Referral for Local Team Services" form to 705-792-2775; Single Plan of Care Coordination - Children and Youth with multiple special needs who have significant mobility issues (use Aid, wheelchair) and or receive Augmentative Communication services who would benefit from a single integrated plan of care * referral by service providers in the Network | |||
