| Description | A safe and supportive alternative for people who would rather be seen in the comfort and safety of their own home than have to access a Regional Hospital or Residential Withdrawal Management Centre * social worker and nurse offer an integrated assessment and treatment plan to assist people in managing their withdrawal symptoms from alcohol/drugs * provides family or support system with the knowledge and support to minimize the risks during withdrawal * daily monitoring of withdrawal symptoms * home visits/in office supportive counselling * 24 hr telephone support * facilitate tampering plan * facilitates family physician involvement * facilitates access to appropriate levels of care i.e. emergency care or medical withdrawal management services if necessary * reduces barriers to obtaining withdrawal management services * reduces risk of withdrawal complications * provides clients with several options of treatment services after they have completed their withdrawal experience | |||
