DescriptionCHATS enhances the health, wellness and independence of seniors and caregivers * offers a full range of in-home and community services that enable seniors to continue living in their own home, such as in-home help and care, Meals on Wheels, transportation, home safety services, wellness/social programs, diversity outreach programs, caregiver support and education, hospital-to-home transition, home medical equipment, and much more * caregiver support and education, transportation, MEDIchair: a home medical equipment store, Lifeline personal response system * Diversity Outreach Programs for isolated seniors of various cultures to support wellness and healthy aging in under-served communities 
 
Tenancy Risk Reduction Service: The program provides support to low and moderate income York Region seniors (or those with an age-related condition) at risk of losing their tenancy due to hoarding behaviour, cluttered living spaces, uncleanliness and squalor, and/or unmaintained homes
