Description (Service) Wellness workshops for individuals with chronic conditions * focus on self-management skills in a small group setting * teaches process for setting goals each week and solving problems individuals living with chronic conditions experience * allows individuals to "tailor the program to own needs * participants take active role in learning self-management, try new ideas, listen to others and share progress with group
